Medical Benefit (Anthem Blue Cross)
- 90% coverage for most in-network services / procedures; 70% for out-of-network after deductible
- $15 in-network office co-pay; $250 deductible (applies to out-of-network or non-illness/injury visits)
- $10 co-pay for generic drugs, $25 for brand name drugs and $40 for non-formulary drugs
- Mail order - 3 month supply of maintenance drugs (1 co-pay for generic drugs, 2 co-pay for brand)
Dental Benefit (MetLife)
- 100% preventive coverage, no deductible, 80% Basic procedures after $50 deductible, 50% for Major
- Annual maximum is $2,000 in-network and out-of-network per each participant
- Orthodontia is covered for children at 50% up to a $1,000 lifetime maximum
Vision Service Plan (VSP)
- One annual exam paid at 100% after $20 deductible; One pair of lenses paid at 100% up to the plan allowance, after $20 deductible; One pair of frames every 12 months up to $130, after $20 deductible
- Contacts annually with a $130 allowance in lieu of a complete pair of glasses
Paid Time Off (PTO)
- 17 days of PTO accrue year 1 - 5; after 5 years, employees accrue 22 days of PTO per year
