  • 90% coverage for most in-network services / procedures; 70% for out-of-network after deductible
  • $15 in-network office co-pay; $250 deductible (applies to out-of-network or non-illness/injury visits)
  • $10 co-pay for generic drugs, $25 for brand name drugs and $40 for non-formulary drugs
  • Mail order - 3 month supply of maintenance drugs (1 co-pay for generic drugs, 2 co-pay for brand)

  • 100% preventive coverage, no deductible, 80% Basic procedures after $50 deductible, 50% for Major
